Who can help me if I have a concern, complaint or grievance?

If an individual or family has any concerns about care and safety at Valley View Hospital or any of its satellite services, he or she is encouraged to contact Valley View Hospital by telephone, email, or by regular mail. Our goal is to promptly respond, address, and remedy any patient or family member concerns. Please feel free to contact us directly regarding any and all concerns.

    • Valley View Hospital is committed to and is required, by law, to keep your Personal Health Information (PHI) private. We are only allowed to share your PHI for treatment, payment and for health care operations. You have the right to let us know if you would like for us not to share your information in certain circumstances. This includes the hospital directory which contains your name, location in our facility, general health condition and religious affiliation. Information in the directory may be disclosed to anyone who asks for you by name unless you ask to be removed from our hospital directory when admitted to the hospital. Valley View Hospital has the responsibility of sharing some PHI with law enforcement when appropriately requested. This includes reasons such as public health and safety activities, victims of abuse, neglect, or domestic violence, health oversight, lawsuits, or when a crime is committed.
