General use:
I give to the Valley View Hospital Foundation, a nonprofit corporation, Tax ID #73-1664673, headquartered in Glenwood Springs, Colorado, the sum of $ (or % of my estate) for its general purposes.
Specific purpose: If you would like to ensure your gift is used for a specific purpose, use this language to indicate your intentions.
I give to the Valley View Hospital Foundation, a nonprofit corporation, Tax ID #73-1664673, headquartered in Glenwood Springs, Colorado, the sum of $ (or % of my estate) for (specify the purpose or department).
We would be honored to recognize your planned gift. If you decide to include the Valley View Foundation in your estate plans, please take the time to let us know by contacting us today. We are also happy to refrain from recognizing your plans publicly, should you choose to remain anonymous.
