Service Availability Form

Valley View patients have the right to have the information necessary to make informed health care decisions. Valley View follows Colorado law (Section 25-58-101, C.R.S.) which requires health facilities to provide patients with information on the availability of specific services and procedures in a service availability form. These specific services and procedures are listed below.
