Health4Life Cardiac Rehabilitation

In our rehabilitation program, our trained professionals work as a team to identify individual patient needs and risk factors. During monitored exercise, we will counsel and educate you on healthy dietary habits, stress management and coping skills. In the end, you will have the tools needed to lead a healthier lifestyle and become confident with returning to your usual daily activities.

Below is a description of the different phases of cardiopulmonary rehabilitation that we offer:

Inpatient program / Phase I
If you are staying in the hospital for heart care, we will answer questions regarding your symptoms/event/procedure. We will help you identify risk factors for future events and introduce methods to modify them. Another major goal is to help you identify signs and symptoms and give you instructions in the case of emergencies. Lastly, we teach you skills to manage home-activity intensity, and bridge you to an outpatient rehab program in your area.

Outpatient Program / Phase II

Patients attend one-hour visits, usually two or three times per week for up to 36 visits. Exercise is guided by physician prescription while feelings of exertion, heart rhythm, and vital signs are monitored by other staff. Regular education and counseling is also provided. We treat the whole person in this phase.
