Valley View’s Health4Life Cardiac Rehabilitation Program
Our cardiac rehabilitation program is medically supervised and specifically intended for anyone living with heart disease or other chronic cardiopulmonary conditions. Our trained professionals work as a team to identify individual patient needs and risk factors.
During monitored exercise, we will counsel and educate you on healthy dietary habits, stress management, coping skills, and more. Our Health4Life program is available for inpatient or outpatient services, known as “Phase I” and “Phase II,” as explained below:
Inpatient Program / Phase I
For patients staying in the hospital for heart care, we provide a Phase I program. Here, we will answer questions regarding your symptoms/event/procedure, help you identify risk factors for future events, and introduce methods to modify them.
We will also help you recognize signs and symptoms, in addition to providing you with detailed instructions on what to do in case of an emergency. Lastly, we will teach you skills to manage home activity intensity and bridge you into an outpatient rehab program one you’re ready.Outpatient Program / Phase II
Our Phase II program includes one-hour visits, usually two or three times per week for up to 36 visits. Exercise is guided by physician prescription while feelings of exertion, heart rhythm, and vital signs are monitored by other members of our staff.
Regular education and counseling are also provided to help you reach your health goals. Most insurance policies cover participation in an outpatient program such as this one.Benefits of Cardiopulmonary Care
