How to Refer a Patient
Monday through Friday, 8 a.m. to 5 p.m. Calls after hours will be sent to an automated system.

Please include the following with your referral:
Supporting documentation to include but not limited to: problem list, past medical history, medication list, demographics, and pertinent clinical information. Please also include all recent: progress note, imaging, biopsy results, labs and surgical interventions. Please utilize a standard referral request form.
Once a referral is received:
- A nurse navigator will contact the referring office with questions
- A nurse navigator will contact the patient
- A nurse navigator share the patient information with Calaway•Young Cancer Center providers
- A nurse navigator will confirm receipt of referral and scheduled appointment with referring office.
